| heal.abstract | Η παρούσα εργασία αποτελεί μια βιβλιογραφική μελέτη της διαχείρισης των τραχειοτομημένων ασθενών με κρανιοεγκεφαλική κάκωση (ΚΕΚ) που βρίσκονται στις μονάδες εντατικής θεραπείας (ΜΕΘ). Για τη μελέτη των συγκεκριμένων ασθενών κρίθηκε αναγκαία η παράθεση του θεωρητικού υπόβαθρου με σκοπό την επεξήγηση των σημαντικών όρων που βρίσκονται στο επίκεντρο της μελέτης. Η εργασία αποτελείται από 5 κεφάλαια. Πιο συγκεκριμένα, το πρώτο κεφάλαιο αναφέρεται στην ανατομία του στοματογναθικού συστήματος και της φαρυγγολαρυγγικής οδού που εξυπηρετεί τόσο τη μεταφορά του βλωμού και την προστασία αεροφόρων οδών από το ενδεχόμενο εισρόφησης, αλλά και τη φώνηση και αντήχηση. Περιγράφονται επίσης τα στάδια της φυσιολογικής κατάποσης. Στο δεύτερο κεφάλαιο γίνεται αναφορά στον μηχανισμό της φυσιολογικής και της παθολογικής κατάποσης αλλά και στις αιτίες που οδηγούν τους ασθενείς να παρουσιάσουν μια διαταραχή της κατάποσης, που αναφέρεται με τον όρο «δυσφαγία». Στο εν λόγω κεφάλαιο γίνεται επίσης αναφορά στον όρο της εισρόφησης και τους κινδύνους που ενέχει για τη ζωή των ασθενών. Το τρίτο κεφάλαιο εμβαθύνει στον όρο της δυσφαγίας. Πιο συγκεκριμένα, η κλινική εκτίμηση της δυσφαγίας, η διαχείριση των ασθενών καθώς και οι μέθοδοι αξιολόγησής της αποτελούν το κεντρικό κορμό του κεφαλαίου. Το τέταρτο κεφάλαιο παρουσιάζει δύο όρους κλειδιά της παρούσας εργασίας, που είναι η ΚΕΚ καθώς και η χειρουργική τραχειοστομία. Περιλαμβάνονται ορισμοί και καίριες πληροφορίες των δύο αυτών όρων, όπως είναι τα εργαλεία μέτρησης συνείδησης μετά από ΚΕΚ και οι πιθανές επιπλοκές της τραχειοστομίας, ενώ παρουσιάζονται και τα χαρακτηριστικά των ασθενών που εντάσσονται σε αυτές τις κατηγορίες. Το πέμπτο και τελευταίο κεφάλαιο της εργασίας είναι αφιερωμένο στη μελέτη της θεραπείας των διαταραχών κατάποσης των τραχειοτομημένων ασθενών με ΚΕΚ. Παρουσιάζονται θεραπευτικές προσεγγίσεις και τεχνικές αποκατάστασης με ιδιαίτερη έμφαση να δίνεται στον ρόλο του λογοθεραπευτή για την διαχείριση των συγκεκριμένων ασθενών. | el |
| heal.abstract | The present bachelor’s thesis is a bibliographic review on the management of tracheostomy patients with traumatic brain injury (TBI) in intensive care units (ICU). For the study of the specific patients, it was deemed necessary to state the theoretical background in order to explain the important terms that are in the focus of the study. The current thesis consists of 5 chapters. More specifically, the first chapter refers to the anatomy of the oral system and the laryngeal tract, which serves both the transport of the bolus and the protection of the airways from the possibility of aspiration, but also the voice and resonance. The stages of normal ingestion are also described. The second chapter refers to the mechanism of normal and abnormal swallowing but also to the causes that lead patients to present a swallowing disorder, referred to as "dysphagia". This chapter also refers to the condition of aspiration and the risks it poses to patients' lives. The third chapter delves into the condition of dysphagia. More specifically, the clinical assessment of dysphagia, the management of patients as well as the methods of its evaluation constitute the central body of the chapter. The fourth chapter presents two key terms of the present work, which is the TBI as well as the surgical tracheostomy. Definitions and key information of these two conditions are included, such as the tools for measuring consciousness after a TBI and the possible complications of tracheostomy, while the characteristics of the patients who fall into these categories are also presented. The fifth and last chapter of the thesis is dedicated to the study of the treatment of swallowing disorders in tracheotomized patients with TBI.
