The human microbiome constitutes a complex and dynamic ecosystem that plays a pivotal role in regulating host physiology and in the development of a wide range of pathological conditions. The present study focuses on the investigation of the relationship between the microbiome and personalized nutrition, highlighting the mechanisms through which dietary factors influence the composition and functional activity of the microbial ecosystem and, consequently, the overall host health. Particular emphasis is placed on the analysis of the structure and function of the microbiome, its interactions with the human host, and the factors that shape microbial diversity. In parallel, the concept of dysbiosis is examined as a state of microbial imbalance, which has been associated with the onset and progression of major diseases, including metabolic syndrome, diabetes mellitus, inflammatory bowel diseases, cardiovascular disorders, as well as neuropsychiatric conditions. Through the synthesis of contemporary scientific evidence, the key pathophysiological mechanisms by which dysbiosis affects the host are analyzed, with emphasis on impaired intestinal barrier integrity, immune dysregulation, and metabolic disturbances. Special attention is also given to the role of microbial metabolites, such as short-chain fatty acids (SCFAs) and trimethylamine N-oxide (TMAO), as mediators linking the microbiome to disease processes. Furthermore, the role of diet as a major modifiable determinant of the microbiome is explored, along with the potential for leveraging this knowledge in the development of personalized nutritional interventions. In addition, the prospects of utilizing the microbiome and its metabolites as biomarkers and therapeutic targets in modern clinical practice are discussed. The findings underline that, despite significant advances in the field, the complexity and inter-individual variability of the microbiome currently limit the development of generalizable applications, highlighting the need for further research and interdisciplinary approaches. Overall, understanding the interaction between the microbiome and nutrition represents a critical step toward more targeted and effective strategies for disease prevention and management.en
