heal.abstractεισαγωγή: οι δυσκολίες στην ακουστική επεξεργασία επηρεάζουν σημαντικά την γλωσσική ανάπτυξη και την κοινωνική επικοινωνία των παιδιών. τα δεδομένα για την φύση των ακουστικών ελλειμμάτων σε άτομα με διαταραχή αυτιστικού φάσματος (δαφ) παραμένουν ετερογενή. σκοπός: η ανασκόπηση σκοπεύει να χαρτογραφήσει την βιβλιογραφία για την διερεύνηση των δυσκολιών ακουστικής επεξεργασίας στα παιδιά με δαφ τυπικής νοημοσύνης μέσω χορήγησης ακουστικών προκλητών δυναμικών και ωτοακουστικών εκπομπών. αυτή είναι η πρώτη μελέτη που θα χαρτογραφήσει τα σχετικά ευρήματα με την χορήγηση όλων των νευροφυσιολογικών μεθόδων σε άτομα με δαφ, ηλικίας 2-18 ετών. μεθοδολογία: η ανασκόπηση ακολούθησε τις αρχές του joanna briggs institute (peters et al., 2021) και του prisma-scr (page et al., 2020). στις βάσεις δεδομένων pubmed, sciencedirect και scopus αναζητήθηκαν ποσοτικές μελέτες (έτος δημοσίευσης >2011) και ελέγχθηκαν οι αναφορές όλων των σχετικών τεκμηρίων για τον εντοπισμό πρόσθετων άρθρων. χρησιμοποιήθηκε το mendeley για την αφαίρεση των διπλότυπων. αποτελέσματα: 31 μελέτες πληρούσαν τα κριτήρια ένταξης και αποκλεισμού της ανασκόπησης. η χορήγηση ακουστικών προκλητών δυναμικών υποδεικνύει αυξημένους λανθάνοντες χρόνους, κυρίως στα κύματα III και V, και επιμήκυνση των διακυματικών διαστημάτων Ι-ΙΙΙ και Ι-V στα παιδιά με δαφ σε σχέση με τους νευροτυπικούς συνομηλίκους, υποδηλώνοντας δυσλειτουργία στο κατώτερο ακουστικό στέλεχος όπως και ατυπίες στην μυελίνωση με ταυτόχρονη όμως ένδειξη διατηρημένης νευρικής πλαστικότητας με την πάροδο της ηλικίας. οι ωτοακουστικές εκπομπές επιβεβαίωσαν την ακεραιότητα των έξω τριχωτών κυττάρων του κοχλία, αλλά παρουσίασαν απουσία της αναμενόμενης καταστολής, υποδηλώνοντας δυσλειτουργία στο έσω ελαιοκοχλιακό σύστημα. τα φλοιώδη δυναμικά και ο αισθητηριακός μηχανισμός, διαπίστωσε μειωμένη νευρική εξοικείωση στα κύματα p1, n2 και απουσία της φυσιολογικής αριστερής ημισφαιρικής εξειδίκευσης για τα γλωσσικά ερεθίσματα στα άτομα με δαφ σε σχέση με τους νευροτυπικούς συνομηλίκους. στην αυτόματη διάκριση μέσω του mismatch negativity-mmn και της μαγνητοεγκεφαλογραφίας εντοπίστηκαν μειωμένο πλάτος και παρατεταμένοι χρόνοι αποκρίσεων στα παιδιά με δαφ που σχετίζονται με έλλειμμα στην αυτόματη αισθητηριακή διάκριση, την επεξεργασία ομιλίας και την αυξημένη αισθητηριακή ευαισθησία ή απόσπαση προσοχής. συμπέρασμα/ συμπεράσματα: τα ευρήματα επιβεβαιώνουν πολυδιάστατες διαταραχές στην ακουστική οδό σε υποφλοιώδες και φλοιώδες επίπεδο στα άτομα με δαφ, εξηγώντας νευροβιολογικά τις αισθητηριακές και γλωσσικές δυσκολίες αυτού του πληθυσμού, χωρίς ωστόσο τα αποτελέσματα των ερευνών να εμφανίζουν καθολικότητα.el
heal.abstractintroduction: difficulties in auditory processing significantly affect children’s language development and social communication. data on the nature of auditory deficits in individuals with autism spectrum disorder (asd) remain heterogeneous. aim: this review aims to map the literature for the investigation of auditory processing difficulties in children with asd of typical intelligence through the administration of auditory evoked potentials and otoacoustic emissions. this is the first study to map relevant findings from administering all neurophysiological methods in individuals with asd aged 2-18 years. methodology: the review followed the principles of the joanna briggs institute (peters et al., 2021) and prisma-scr (page et al., 2020). we searched pubmed, sciencedirect, and scopus for quantitative studies (publication year >2011), and checked the references of all relevant documents to identify additional articles. mendeley was used to remove duplicates. results: 31 studies met this review’s inclusion and exclusion criteria. the administration of auditory evoked potentials indicates increased latencies, mainly in waves iii and v, and elongation of interpeak intervals i-iii and i-v in children with asd compared to neurotypical peers, suggesting dysfunction in the lower auditory brainstem as well as atypies in myelination, with a simultaneous indication, however, of preserved neural plasticity over age. otoacoustic emissions confirmed the integrity of the cochlea’s outer hair cells, but presented an absence of the expected suppression, suggesting dysfunction in the medial olivocochlear system. cortical potentials and the sensory mechanism found reduced neural habituation in waves p1 and n2 and an absence of the normal left hemispheric specialization for language stimuli in individuals with asd compared to neurotypical peers. in automatic discrimination through mismatch negativity-mmn and magnetoencephalography, reduced amplitude and prolonged response times were identified in children with asd, which are associated with a deficit in automatic sensory discrimination, speech processing, and increased sensory sensitivity or distractibility. conclusion/conclusions: the findings confirm multidimensional disorders in the auditory pathway at the subcortical and cortical level in individuals with asd, neurobiologically explaining the sensory and language difficulties of this population; however, the findings are not universal.en
