| heal.abstract | Η παρούσα διπλωματική εργασία αφορά την μελέτη της διαγνωστικής ακρίβειας των εμπορικά διαθέσιμων τεστ βιοδεικτών ούρων για την υποτροπή του καρκίνου της ουροδόχου κύστης. Στις εμπορικά διαθέσιμες δοκιμές για επιτήρηση του συγκεκριμένου καρκίνου που δεν είναι εγκεκριμένοι από τον Παγκόσμιο Οργανισμό Υγείας, συμπεριλαμβάνονται οι εξής: ADXbladder, Bladder Epicheck, Cxbladder Monitor, Uromonitor και Xpert Bladder Cancer. Οι διαγνωστικές τους ικανότητες και οι μελέτες στις οποίες εμφανίζονται εντοπίστηκαν μετά από αναζήτηση στις βάσεις δεδομένων, PubMed και EMBASE. Εξήχθησαν και αξιολογήθηκαν 307 μελέτες, από τις οποίες οι 41 πληρούσαν τα κριτήρια επιλεξιμότητας. Όλες οι μελέτες που συμπεριλήφθηκαν στην μετα-ανάλυση διερεύνησαν την υποτροπή του καρκίνου ουροδόχου κύστης σε ασθενείς που είχαν ήδη διαγνωσθεί για τον συγκεκριμένο καρκίνο. Μετά το πέρας της επιλογής των μελετών, ακολούθησε η εξαγωγή των δεδομένων από τις επιμέρους μελέτες για κάθε διαγνωστικό τεστ. Το CxBladder Monitor περιείχε μόνο μια μελέτη που συμφωνούσε με τα κριτήρια επιλογής, χωρίς να δύναται να πραγματοποιηθεί η μετα-ανάλυση του. Επομένως, πραγματοποιήθηκαν τέσσερις μετα-αναλύσεις, μία για κάθε τεστ. Σε αυτές επιλέχθηκαν οι μελέτες που παρουσίασαν την συνολική διαγνωστική εκτίμηση του τεστ αναλύοντας τον συνολικό πληθυσμό και όχι συγκεκριμένες ομάδες με βάση τον βαθμό ή το στάδιο του καρκίνου. Το Xpert Bladder Cancer και το Bladder Epicheck συμπεριέλαβαν στις μετα-αναλύσεις τους 13 και 10 μελέτες, αντίστοιχα. Εξάγοντας συγκεντρωτικές τιμές για την ευαισθησία (διάστημα εμπιστοσύνης) και ειδικότητα (διάστημα εμπιστοσύνης) ίσες με 0.71 (0.63, 0.77), και 0.80 (0.75, 0.84), 0.71 (0.63, 0.78) και 0.87 (0.82, 0.90), αντίστοιχα για κάθε τεστ. Επιπλέον, τα τεστ ADXbladder και Uromonitor πραγματοποίησαν την μετα-ανάλυση τους με πέντε και τρείς μελέτες, αντίστοιχα. Τα αποτελέσματα έδειξαν ότι οι συγκεντρωτικές τιμές ευαισθησίας και ειδικότητας με τα αντίστοιχα διαστήματα εμπιστοσύνης είναι ίσες με 0.52 (0.37, 0.66) και 0.67 (0.54, 0.77), 0.86 (0.66, 0.95) και 0.96 (0.87, 0.99). | el |
| heal.abstract | This master thesis concerns the study of the diagnostic accuracy of commercially available urine biomarker tests for bladder cancer recurrence. Commercially available tests for surveillance of this specific cancer that are not approved by the World Health Organization include the following: ADXbladder, Bladder Epicheck, Cxbladder Monitor, Uromonitor and Xpert Bladder Cancer. Their diagnostic performance and the studies in which they appear were identified after searching the databases, PubMed and EMBASE. 307 studies were extracted and evaluated, of which 41 met the eligibility criteria. All studies included in the meta-analysis investigated bladder cancer recurrence in patients who had already been diagnosed with this cancer. After the selection of the studies, the data from the individual studies for each diagnostic test were extracted. CxBladder Monitor contained only one study that met the inclusion criteria, and its meta-analysis could not be performed. Therefore, four meta-analyses were performed, one for each test. These selected studies that presented the overall diagnostic assessment of the test by analysing the total population and not specific groups based on cancer grade or stage. Xpert Bladder Cancer and Bladder Epicheck included 13 and 10 studies in their meta-analyses, respectively. Extracting pooled values for sensitivity (confidence interval) and specificity (confidence interval) equal to 0.71 (0.63, 0.77), and 0.80 (0.75, 0.84), 0.71 (0.63, 0.78) and 0.87 (0.82, 0.90), respectively, for each test. In addition, the ADXbladder and Uromonitor tests performed their meta-analysis with five and three studies, respectively.
