| heal.abstract | Στις σύγχρονες ΜΕΘ προκύπτουν, συχνά, κρίσιμα βιοηθικά διλήμματα λόγω αύξησης των ασθενών με πολλές συνοσηρότητες αλλά και της δυνατότητας υποστήριξης και παράτασης της ζωής με σύγχρονα τεχνολογικά μέσα. Τα κρίσιμα βιοηθικά ζητήματα που ανακύπτουν στη διάρκεια νοσηλείας ασθενών στις ΜΕΘ περιλαμβάνουν: α. την εισαγωγή ενός ασθενή στη ΜΕΘ, που καθορίζεται από την ωφελιμότητα που αναμένεται, από τη συναίνεση του ασθενή ή των συγγενών του, αλλά και από τις εκάστοτε διαθέσιμες κλίνες νοσηλείας στη ΜΕΘ. β. τη μη κλιμάκωση (whithholding) ή και την απόσυρση (withdrawing) της θεραπευτικής αγωγής, όταν αυτή δεν επιτυγχάνει τους στόχους που έχουν τεθεί εκ των προτέρων. γ. την επίκληση, εκ μέρους του ιατρού, της ιατρικής ματαιότητας (medical futility), δηλαδή, του ανώφελου μιας θεραπευτικής παρέμβασης. δ. την εφαρμογή μιας ιατρικής παρέμβασης που μπορεί να έχει, ταυτόχρονα, δυο αποτελέσματα σημαντικά, αλλά, εντελώς αντίθετα: ένα ωφέλιμο και ένα καταστροφικό. Πρόκειται για την αρχής του διπλού αποτελέσματος (principle of double effect). Στην εποχή μας ολοένα και περισσότερο τα παραπάνω ζητήματα τείνουν να επιλύονται μέσω της συν-απόφασης (shared decision making) μεταξύ του προσωπικού της ΜΕΘ και των ασθενών ή των συγγενών τους. | el |
| heal.abstract | In modern ICUs, critical bioethical dilemmas often arise due to the increase of patients with many comorbidities but also the possibility of support and extension of life with modern technological means. Critical bioethical issues that arise during the hospitalization of patients in ICUs include: a. the admission of a patient to the ICU, which is determined by the expected benefit, by the consent of the patient or his relatives, but also by the available beds in the ICU. b. the non-escalation (whithholding) or even the withdrawal (withdrawal) of the treatment, when it does not achieve the goals that have been set in advance. c. the invocation, on the part of the doctor, of medical futility, that is, of the uselessness of a therapeutic intervention. d. the implementation of a medical intervention that can have, at the same time, two important results, but, quite opposite: one beneficial and one destructive. This is the principle of double effect. Nowadays, the above issues tend to be resolved through co-decision (shared decision making) between ICU staff and patients or their relatives. | en |
