Dimografic study of first episode of psychosis at University Hospital of Ioannina (Master thesis)
In recent years there is a growing interest from clinicians, investigators and health policy makers on the first episode of psychosis (FEP). As a result, a large number of studies on pathophysiology, etiology and treatment, even from the prodromal stages of a psychotic disorder have been published. Our classic knowledge for the symptomatology of such disorders has been enhanced by the contribution of genetics and neurosciences, which facilitate understanding and treatment of these syndromes. Currently available treatments are not completely satisfactory and the prevailing view is that interventions should be applied as early as possible after the onset of a FEP and to be multimodal, that is the implementation of drug treatment, as well as psychological and psychosocial treatment. It is widely accepted that early intervention improves prognosis and facilitates recovery. Prognosis of psychotic disorders remains moderate and outcome is impeded by symptomatology, high rates of alcohol/substance abuse, and poor treatment adherence and disengagement from services. The last two decades there is a tendency toward interventions at the perceived as “high risk” population. This is strongly supported by several investigators, but others have criticized this tendency, which is uncertain whether can be effective when implemented in routine clinical practice. Moreover, there have been raised important ethical concerns. Despite the ongoing debate, several early intervention services have been developed worldwide and their work is published at scientific journals.
